ADHDcentraal’s use of personal data.
ADHDcentraal obtains personal data from you, for example the data you provide to us through the website, email, phone and app. In addition, we may obtain your personal data through third parties in connection with our services. Consider, for example, a referral letter from your primary care physician or other practitioner/referrer.

Foundations
We process said personal data to perform the agreement with you, or to comply with a legal obligation, such as, for example, obligations under the Medical Treatment Agreement Act.
If personal data are processed pursuant to (explicit) consent, this will be requested separately.
Provision to third parties
In the course of providing its services, ADHDcentraal may exchange personal information. ADHDcentraal may use the services of third parties for the aforementioned purposes, such as the IT suppliers of our website and our systems. Personal data are provided to these third parties as part of this process. These third parties may only process your personal data for the aforementioned purposes.
Finally, your personal data may be provided to third parties when ADHDcentraal must comply with a legal obligation. For purposes of funding, data will be sent to your health insurance company. ADHDcentraal will not provide your information for commercial or charitable purposes.
On your own initiative, you may object to the sharing of data traceable to a diagnosis and/or care demand typing set by the health care provider with respect to you as a patient. To do this, you and your practitioner can complete a privacy statement Care Performance Model.
Transfer of your data outside the EEA
Your personal data will be processed within the EEA. The EEA is the European Economic Area, consisting of the countries of the European Union, Liechtenstein, Norway and Iceland.
How long we keep your data
ADHDcentraal will not process your personal data for longer than necessary for the purposes stated in this privacy statement. This means that your personal data will be kept as long as they are necessary to achieve the purposes in question Certain data do need to be kept longer because ADHDcentraal must comply with legal retention obligations, such as tax or medical retention obligations (e.g. medical data patient file for 20 years, Medical Treatment Agreement Act).
How we secure your information
We believe it is important that your personal information is protected against loss or unauthorized access of your personal information. Therefore, ADHDcentraal has taken appropriate organizational and technical security measures.
Profiling
We conduct behavioral analyses of the information we collect about you. We do this in order to improve our services and care or to meet the requirements of health insurance companies. This information is anonymized.

Cookies are used on the website to improve online services. Cookies process your IP address and your device’s system information.
Your rights
You have the right to request ADHDcentraal to review your personal information. Upon receipt of your request, you will receive an overview of your personal data within 1 month. If this reveals inaccuracies, you may request that your data be amended, supplemented, deleted or blocked.
You may also request ADHDcentraal to transfer your personal data or object to the processing of your personal data due to special personal circumstances.

To prevent abuse of these rights, when submitting your request we ask you to identify yourself by sending a copy of a valid ID. Then remember to make your BSN and your passport photo illegible on the copy. For example, you can use the ‘KopieID’ app for this purpose.
If you have a complaint about the processing of your personal data by ADHDcentraal, please let us know. If you cannot work it out with us, you have the right to file a complaint with the privacy regulator, the Authority for Personal Data. You can contact the Personal Data Authority for this purpose.

Privacy regarding delivery of care demand typing to the Dutch Healthcare Authority (NZa)
As a healthcare provider, ADHDcentraal is required to submit the care demand typing to the NZa. They consider the data retrieval for care demand typing important on the assumption that it will allow them to help people in need of mental health (mental health) services as quickly as possible and offer care providers appropriate reimbursement.
The NZa indicates that it handles the supplied data with care. They use the dataset to improve healthcare demand typing and contracting between healthcare providers and insurers.
